Our ultimate goal is to improve our participants’ overall quality of life. We offer three evidence-based and disease prevention programs that assist older adults in preventing illness and managing chronic conditions. In each one of the programs, we cover topics which promote healthy aging and maintenance of optimal physical, mental, and social well-being in older adults.
These free programs are available throughout San Francisco in English, Spanish, and Chinese (Cantonese). Our workshops are available in-person and virtually.

Healthier Living, otherwise known as Chronic Disease Self-Management Program (CDSMP) is a 6 weeks workshop developed by Self-Management Resource Center (SMRC) from Stanford University School of Medicine that focuses on problems adults with any chronic condition face and builds self-efficacy while providing tools, non-medical in nature, ranging from setting goals, problem-solving, mind-body techniques, better breathing, healthy eating, physical activity, and better communication.
The Diabetes Empowerment Education Program (DEEP) is developed by the Midwest Latino Health Research Training and Policy Center at the University of Illinois at Chicago. and available to individuals who are diabetic, pre-diabetic and seniors interested in prevention. This 6 weeks workshop focuses on increasing self-management skills to decrease diabetes risk factors, including obesity and hypertension, and increase your knowledge of diabetes.
Chronic Pain Self-Management Program (CPSMP) is a
6 weeks workshop developed by Self-Management Resource Center (SMRC) from Stanford University School of Medicine that focuses on problems adults with chronic pain face and build self-efficacy while providing techniques, ranging from setting goals, building confidence, moving easy exercise, better breathing, healthy eating, physical activity, and the importance of good sleep.
The workshops are offered to older adults (60+ years old) who live in the City and County of San Francisco for free but donations are welcomed.
